Abstract:

Objective: to investigate the vision of health professionals regarding the coordination of care in Primary Health Care (PHC) and the challenges of exercising. Methods: qualitative research with professionalstwo teams of PHC that work in territories with great vulnerabilities and inequalities in the city of Rio de Janeiro. Conducting interviews with semi-structured script and participant observation; use content analysis. Results: coordinating care, to health professionals, is to be involved with the activities that provide the provision of an individualized and comprehensive care aimed at the continuity of care. The challenges to coordinating PHC care are to overcome the fragmentation of the network, the low number of vacancies for specialized services, the fragile communication between services, the non-integrated electronic medical record, the low professional qualification and the lack of knowledge of the role of PHC by other levels of care. Conclusions: The strengthening of PHC as a gateway, care coordinator and network organizer requires broad efforts by managers, professionals and society, so that this mode of system organization can bring benefits in terms of equity, accessibility, effectiveness clinical and sanitary conditions, economic efficiency and the integration of health care.
