Abstract:

This cross-sectional study presents the rate and examines the rule of socio-demographic factors, health conditions, health system characteristics and contextual factors on cost-related medication nonadherence (CRN) among older women (age &#8805; 70). Participants in the Greater Metropolitan Belo Horizonte Health Survey (GMBH) and in the eleventh wave of Bambuí Cohort Study of Ageing formed the subset for analysis (n=478 and n=535, respectively). Among older women residing in the GMBH, the prevalence rate of CRN was 11.4%, and for those residing in Bambuí, this rate was 5.4%. Poor health status (OR, 3.46; 95%CI, 1.32–9.10); any ADL limitation (OR, 2.75; 95% CI, 1.31-5.78) and no perception of help (OR, 2.36; 95% CI, 1.07-5.25) had the strongest effect on the likelihood of CRN among GMBH residents. Contrasting, low perception of attachment to the neighborhood (OR, 2.38; 95% CI, 1.02-5.56) and low perception of physical environment (OR, 2.58; 95% CI, 1.10-6.03) increased significantly the likelihood of CRN among Bambuí residents. These results provide important clues to identifying possible risk factors for CRN, highlighting the need to develop strategies targeting the amplification of the involvement between the older women and their community to reduce the extent of CRN in later life.
