Abstract:

This research studied the discontinuity of oral health care (DC) among children and adolescents who accessed the emergency services at the basic health units (UBS) and emergency care units (UPA). The records of patients aged 0 to 17 years treated at the Unified Health System of Curitiba, Brazil, were reviewed. DC was considered when elective treatment was not registered at the UBS within 6 months after urgent care. The sample was stratified by the Municipal Human Development Index (MHDI) of Human Development Units (HDU) into ≤ 0.799 or ≥ 0.800. The association between DC and the other covariables was assessed by a univariate and multivariate Poisson regression model with robust variance (α=0.05). The incidence of DC was 42.2%. For an MHDI ≤ 0.799 stratum, the risk of DC was greater among children younger than 5 years and among those who had not had a dental appointment in the previous year. At both MHDI strata, the risk of DC was higher in those patients who received urgent care at UPA than in those who were treated at UBS. Discontinuity of oral health care had a high incidence and, among those individuals living in regions with a lower MHDI, it was strongly influenced by the characteristics of dental care use.
