Este artigo analisa os avanços e desafios da atenção à saúde da população idosa, sobretudo daquela com doenças crônicas na atenção primária, tendo como cenário de estudo uma clínica da família na cidade do Rio de Janeiro. Os dados foram produzidos a partir da realização das técnicas de observação participante, grupo focal com equipe da ESF, entrevista com o gestor da clínica da família e entrevistas com idosos acompanhados ou não de familiares ou cuidadores. Destaca-se o deslocamento do modelo de atenção à saúde de caráter biomédico para o modelo biopsicossocial e suas implicações na atenção, prevenção e promoção da saúde do idoso. Apesar dos avanços observados na assistência, dificuldades, sobretudo em relação ao acesso da população ao serviço foram observados apontando para a perpetuação de iniquidades no cuidado à saúde. Quanto aos idosos com doenças crônicas, nota-se que a equipe de ESF lança mão de uma série de estratégias tanto individuais quanto coletivas, cujos efeitos foram identificados nas falas dos idosos, familiares e cuidadores, que qualificam de modo positivo a assistência recebida. Conclui-se que o processo de cuidado é influenciado por uma miríade de fatores e que se configuram como objetos de questionamento e intervenção no âmbito da atenção primária.

Abstract:
This article presents and discusses the developments and challenges to the elderly population health care, especially those with chronic diseases in primary care, having as a study scenario a family clinic in the city of Rio de Janeiro. The data were produced through the development of techniques such as participant observation, a focal group with an ESF team, an interview with the family clinic manager, and interviews with the elderly with or without relatives or caregivers. It is highlighted the biomedical health care model displacement for the biopsychosocial model and its implications in the attention, prevention and health promotion of the elderly. Despite the advances observed in the assistance, difficulties, especially in relation to the access of the population to the service were observed pointing to the perpetuation of inequities in health care. As for the elderly with chronic diseases, it is noted that the ESF team uses a series of strategies, both individual and collective, whose effects were identified in the speeches of the elderly, family members and caregivers, who qualify the assistance received. It is concluded that the care process is influenced by a myriad of factors and that are configured as objects of questioning and intervention in primary care
