O objetivo foi analisar a validade e acurácia do Índice de Massa Corporal (IMC) e Circunferência da Cintura (CC) no do excesso de gordura corporal (GC) e propor pontos de corte mais adequados para idosos. Os pontos propostos da literatura IMC(25,27 ou 30kg/m2) e CC (?102cm-homens e ?88cm-mulheres ou ?90cm-homens e ?80cm-mulheres) foram avaliados conforme densitometria corporal total. A elevada GC foi definida por percentil >90. Dos 132 idosos, 60,6% eram mulheres de 60-91 anos. Os pontos de corte recomendados de IMC? 25 e ?27kg/m2 apresentaram desempenhos similares, sendo que o IMC?30kg/m2 apresentou alta especificidade e baixa sensibilidade no sexo masculino e melhor desempenho para GC nas mulheres. Os pontos de corte convencionais de CC não apresentaram boa sensibilidade/especificidade. Foram propostos novos pontos de corte para IMC(masculino-25kg/m2; feminino-26,6kg/m2) e CC (masculino-98,8cm; feminino-77,8cm). Estes valores demonstraram sensibilidade e especificidade>74% e acurácia >76%. As áreas sob a curva foram >0,86. O presente estudo propõe a utilização de pontos de corte mais acurados para diagnóstico de GC em idosos, sendo para homens IMC 25kg/m2 e CC de 98,8cm e para mulheres IMC de 26,6kg/m2 e CC 77,8cm, com melhores níveis de sensibilidade e especificidade.

The objeticve was to analyze the vality and accuracy of the Body Mass Index (BMI) and Waist Circumference (WC) in evaluation of excess body fat and identifying cutoff points more adequate for the elderly. The recommended cut points for BMI (25, 27 or 30kg / m2) and WC (≥102cm men and ≥88cm women or ≥90cm men and ≥80cm women) were evaluated and compared to total body densitometry, being overweight when >90th percentile. From of 132 elderly studied, 60.6% were women and the age group of 60-91 years. The recommended cut-off points of IMC≥25kg/m2 and IMC≥27 kg/m2 presented similar performances, and IMC≥30 kg/m2 presented high specificity, but low sensitivity in males and better performance for body fat in women. Conventional WC cut-off points did not show good sensitivity and specificity. New cut-off points for BMI (male-25 kg/m2, female-26.6 kg/m2) and WC (male-98.8cm, female-77.8cm) were proposed. These values showed sensitivity and specificity >74% and accuracy >76%. The areas under the curve (ROC) were >0.86. The present study proposes more accurate cutoff points for the diagnosis of body fat in the elderlyBMI and WC (male- 25 kg/m2 and 98.8 cm and female- 26.6 kg/m2 and 77.8cm) the best levels of sensitivity and specificity presented.
