0234/2023 - Organização da Atenção Primária paulista no enfrentamento da Covid-19: análise segundo porte populacional São Paulo State Primary Health Care in coping with Covid-19: population size analysis.
O presente artigo tem como objetivo compreender como os municípios paulistas organizaram o enfrentamento da pandemia de Covid-19, destacando o papel da Atenção Primária à Saúde como analisador do modelo de atenção. Trata-se de um estudo quantitativo descritivo a partir da realização de inquérito com uma amostra probabilística de 253 municípios do estado de São Paulo no qual foram entrevistados gestores municipais por meio de questionário. A descrição das frequências absolutas (n) e relativas (%) foi feita após ponderação segundo os três estratos de portes populacionais. Os resultados indicam o porte populacional como importante componente analítico. A organização durante a pandemia priorizou reajuste de fluxo e cuidados clínicos. As ações continuadas de pré-natal e puericultura apresentavam caráter biomédico. Em relação ao fomento de cuidado ampliado e territorial, os municípios de menor porte, com centralidade na APS, tiveram um desempenho superior. Já os municípios de grande porte fragmentaram o cuidado e a vacinação. Conclui-se que as ações intersetoriais, de cuidado comunitário, com perspectiva territorial, mostraram-se retraídas e que persistem dificuldades da APS em centralizar as redes e em implementar modelos de atenção mais abrangentes.

Abstract:
This article aims to understand how the cities of São Paulo state organized the coping with the Covid-19 pandemic, highlighting the role of Primary Health Care (PHC) as an analyzing element of the healthcare model. It is a descriptive quantitative study based on a survey with a probabilistic sample of 253 municipalities in the state of São Paulo in which municipal managers were interviewed through a questionnaire. Absolute (n) and relative (%) frequencies description was made after weighting according to the three population strata. The results indicate the population size as an essential analytical component. The organization made during the pandemic prioritized clinic care and flow readjustment. The prenatal care and childcare continued actions consisted in biomedical nature actions, with medical consultations. Regarding the promotion of expanded healthcare responses, the smaller municipalities, which are structured based on the PHC, performed better. Large cities, on the other hand, split healthcare and vaccination. This study concludes that intersectoral activities, community care actions, and efforts using the territorial perspective were still retracted and remains the difficulty of a PHC ordering healthcare networks and in implementing more comprehensive care models.
