A desinstitucionalização promove a retirada de usuários dos manicômios com o apoio da Rede de Atenção Psicossocial (RAPS), que tem os Centros de Atenção Psicossocial (CAPS) como base. Um dos principais impasses atuais é a emergência de instituições particulares/religiosas que reatualizam o modelo manicomial. Para analisar o que leva à neoinstitucionalização nestes espaços, foi feita uma etnografia sobre nove casos de quatro CAPS das três RAPS do município do Rio de Janeiro, por meio de leituras de prontuários, entrevistas com o paciente, familiares, técnicos e gestores do CAPS e da instituição. Estes usuários foram institucionalizados apesar do acompanhamento no CAPS, sendo selecionados para a pesquisa devido à diversidade de características, territórios e destinos institucionais que permitiram investigar. Concluiu-se que a neoinstitucionalização é motivada por diferentes fatores: crise, imaginário manicomial, desarticulação e precarização das redes intra e intersetorial, equipes reduzidas e número insuficiente de serviços territoriais, institucionalização dos CAPS, baixa capacidade de acolhimento à crise, sobrecarga familiar e expectativa sobre a família, falta de suporte regular aos familiares e de projetos de moradia para usuários cujos cuidadores morreram, envelheceram ou adoeceram.

One of the main current impasses is the emergence of private/religious institutions that re-update the asylum model. To analyze what leads to neoinstitutionalization in these spaces, an ethnography was carried out on nine casesfour CAPS of the three RAPS in the city of Rio de Janeiro, through readings of medical records, interviews with the patient, family members, CAPS technicians and managers and of the institution. These users were institutionalized despite the support of the CAPS, being ed for the research due to the diversity of characteristics, territories and institutional destinations that could be investigated trough their cases. It was concluded that neo-institutionalization is motivated by different factors: crisis, asylum imaginary, disarticulation and precariousness of intra- and intersectoral networks, reduced teams and insufficient number of territorial services, institutionalization of CAPS, low crisis reception capacity, family overload and expectations on the family, lack of regular support for family members and housing projects for users whose caregivers have died, aged or become ill.
