Abstract:

Psychiatric reform addressed individual guarantees, restricted non-voluntary hospitalizations, created disincentives to psychiatric hospitalizations and developed local services network. The diversification of users and Health Minister regulations adapted to support and needs of individuals and families and to growing demands set by drug users care agenda. The shortage in local public services favored diffusion of services of private services with clinical guidelines not validated and the debate over non-voluntary hospitalizations. We analyze the delivery of health care in alcohol, crack and others drugs in the Municipality of Rio de Janeiro and noted access gaps in specialized and 24 hours services and the psychiatric shortage in multi professional teams. There is an experimental diffusion of private services, religious or not, and of public shelters in response to new demands. The process analyzed reveals an experimental path in sectorial policy and recommendations are made about governmental policies.
