Abstract:

The paper aims to compare practices of care in healthy newborns at the time of birth inpublic and mixed hospitals of the unified health system (SUS), according to the type of birth, in “Birth in Brazil-2011” (BB) study and in the last cycle of the “Stork Network Evaluation-2017” (SNE) study. The NB study included a sample of 266 hospitals with national representativeness, and the SNE studyincluded all 606 maternity hospitals included in the “Stork Network” strategy, totaling 15,994 and 8,047 pairs of mothers and healthy newborns, respectively. Between the two studies, BB-2011and SNE-2017, although the proportion of cesarean sections remained around 44%, the prevalence of skin-to-skin contact with the newborn, breastfeeding in the delivery room and breastfeeding in the first 24 hours of life increased in 140%, 82% and 6%, respectively. The proportion of upper airways aspiration of the newborn, an intervention recognized as unnecessary, dropped by 65%.
The results indicate that the use of evidence-based guidelines for care of healthy newborns increased in clinical practice, considering the six-year period between the compared studies. In spite of this progress, important challenges remain so that best practices are guaranteed for the totality of women and newborns, particularly in relation to cesarean section births.
