Abstract:

We have attempted to identify the boundaries of the autonomy of local government in the context of the federal pact in the Brazilian National Health System and build the case for such governments to explore institutional autonomy trough innovation, creativity and evidence-informed decision making. We have asked questions that favored dialogue with specific literature to analyze the influence of centrally formulated policies in spaces of local autonomy and its boundaries considering current legal norms. We have then presented ideas and examples of how to qualify the local decision-making process. We verified a gradual reduction in municipal autonomy due to the rising financial commitment undertaken by local governments –partially reversed by the Health Agreement (Pacto pela Saúde/2006) and the increased ties attached to federal transfer to municipalities. We described new parameters of local health agenda accrued by the Decree 7508/2011 and emphasized the importance to optimize decision-making processes through initiatives aiming at increasing innovation, creativity, and evidence-informed policy making.
