Abstract:

According to current global AIDS policy, the anti-HIV test is crucial to the success of the \'treatment as prevention\' (TasP) strategy and for the control of AIDS. In view of Brazil\'s commitment to this policy, this article aims to characterize the principles and justifications of TasP and discuss the challenges of its implementation. The analysis is based on a systematic review of the academic literature (2005 to 2015) about strategies to outreach and to provide testing to men who have sex with men (MSM). This approach is found on the assumption that academic production is a relevant source for understanding the foundations and appropriations of global AIDS policies in local contexts. According to the analysis of the 65 articles ed, TasP operates a transformation in the preventive paradigm, suggesting a superposition between prevention and care, assigning greater weight to biomedical knowledge and practice. This approach does not address the confrontation of structural factors related to HIV vulnerability and AIDS-related stigma and the participation of activists and PLHA as autonomous producers of preventive practices. We argue that the effectiveness of TasP in Brazil requires a discussion about the guarantee of human rights and the structural and programmatic problems of the public health system.
