Abstract:

The discontinuity of care and actions in the territory in primary health care during pandemic periods can lead to an increase in acute and chronic complications and mortality. In this regard, this article aims to present the barriers and facilitators for the continuity of care and actions in the territory during the COVID-19 pandemic in municipalities in the state of São Paulo. It is a qualitative analysis of 37 interviews with health managers and professionalssix municipalities. Two facilitating factors were identified: the existence of a specific service for attending respiratory demands related to COVID-19 and the integration of the health team professionals before the pandemic. Two barriers were evident: a fragile primary health care structure before the pandemic and the lack of alignment of health professionals\' actions with the community and territorial care model. It was concluded that municipalities that already had a structured primary health care system before the pandemic faced fewer difficulties in maintaining the continuity of care and actions in the territory.
