Abstract:

The study reports the development and initial validity of a Social Vulnerability Index-SVI and demonstrate the SVI application to inform equity in allocation of resources of the Brazilian National Health System (SUS) for primary care among the municipalities in the Brazilian State of Rio Grande do Sul. The SVI items were generated using data on population poverty and population density from the 2010-population census of the Brazilian Institute of Geography and Statistics. Exploratory factor analysis suggested an unidimensional scale including five items with high factor loadings in the first principal component; the scale was saved as Z-scores. Also in confirmatory factor analysis, the unidimensional structure showed good fit to the data. The SVI-5 showed acceptable internal consistency reliability. External validity was demonstrated by higher rates of infant mortality and hospitalization due to ambulatory care sensitive conditions among the most vulnerable municipalities. The SVI-5 was used for resource allocation to primary health care in Rio Grande do Sul in 2012 and 2013. The results suggest that the SVI-5 is a valid measure social vulnerability, potencially useful for policies directed to equity in health in the Brazilian National Health System (SUS).
