Abstract:

The study aimed to identify the areas of greater difficulty of access to specialized consultations offered by SUS in small cities in the 18th Regional Administration of Health Care of Paraná, Brazil using case study methodology. The data were collected by interviews with managers and those responsible for regulation of specialized consultations and also the Managing Director of the CIS. The 21 specialties studied were rated like Sufficient Share, Insufficient Share, Inexistent Supply and Assistance Gap. The specialties presenting more difficulties were the following: Vascular, Proctology, Geriatrics, Endocrinology and Neurology considered Inexistent Supply/Assistance Gap, and Orthopedics, Neuro-paediatrics, Urology, Rheumatology, Ophthalmology and Otorhinolaryngology, considered Insufficient Share. Other issues contribute to the magnitude of the problem: shortage of specialist doctors resulting in insufficient supply to consultations; dependence of the SUS to private sector and decrease in the participation of the Union and the State in financing the Health System. Therefore the gap in the specialized healthcare is complex and very difficult to solve in short and medium-term, proving that the Medium Care is the “bottleneck” of Brazilian Health Care.
