Abstract:

Children discharged from Neonatal Units require specialized monitoring due to the complexity of the care. There is a lack of follow-up services after discharge from Neonatal Units and a lack of integration between the different levels of care, demonstrating a lack of institutional relations between Primary Care and Hospital Care. There are well-structured follow-up clinics and well-organized Basic Health Units, but this is not the reality throughout the country. Considering this, a proposal was drawn up to organize care after hospital discharge with a hierarchical care system, integrated by a network of outpatient clinics of different complexities according to the care to be provided, allowing for the optimization of resources. The systematization of the network, integrating hospital care, specialized outpatient clinics and Primary Care, with specific attributions for each level, can optimize the existing infrastructure resources and professionals for performing diagnoses and indicated therapies, supporting the implementation of this outpatient service, established in the Alyne Network, with benefits for the children served. If complemented by recording the progress of these children, it can provide useful information for the formulation of public policies by the SUS.
