Objective: Analyze the implementation of the strategy Directly Observed Treatment Short course (DOTS) in the control of tuberculosis, under the view of the coordinators of the Tuberculosis Control Program (PCT) in six municipalities of priority in Paraíba, Brazil. Methods: Semi-structure interviews were carried out with seven coordinators of PCT. Results: DOTS fortresses: five municipalities reached 90% of cure. Among the DOTS debilities were identified, in the political dimension, the discontinuity of the coordinator position of the PCT, the unpreparedness of the local team, precarious technical-administrant structure and insufficiency of the laboratorial network. In the operational dimension is still low the search for respiratory symptoms by the Family Health Teams. Discussion: Changes of epidemiological, operational and political character. Conclusion: The implementation and guarantee of the sustainability of DOTS in the State depend on the way the health services are organized and on the manager’s political compromise to support the strategy.Key words: Tuberculosis; Directly Observed Therapy, Family Health; Health Care Systems
