Abstract:

The article aims to understand how the National Policy on Men’s Health Care (PNAISH) arrives at the Primary Care Health Services, from the standpoint of health professionals and by observing their daily practices. A case study, involving five cases (each of a different Region of Brazil), carried out by employing ethnographic techniques of data collection, allows us to map the course of the PNAISH. Analysis is set within the anthropological framework and employs Lipsky’s idea of street-level bureaucreacy. The formulation of PNAISH is contextualized in historical terms and in terms of the production of national and international documents. Recognizing that there will always be a gap between the formulation and the implementation of any policy, we seek to understand how such a distance has been built by analyzing the translation of the PNAISH into city Action Plans (PAs). Implementing agents had little knowledge of the PNAISH, of the local health care network for men, of the techniques required to meet the specific needs of men and of the concept of gender. The implementation of the PNAISH encountered institutional obstacles, such as lack of an organizational structure, of a consolidated network of care – where the user is served by services with different degrees of complexity within the system – and of resources in general, especially human resources.
