Abstract:

We studied mental health policies in Brazil as to normative, demand and supply side and financing aspects. We concluded that sustainability of psychiatric innovations depend on better financing and integration with primary care communitarian services, on SUS performance and on reducing apart and exclusive services in primary care. We point to high and increasing demand stress over services measured in DALY and burden of disease. Observed diminishing psychiatric beds followed systemic reduction though selective to psychiatric hospitalizations. CAPS services show institutional problems due to civil service organizational model adopted and local government capacity. Available secondary data shows that: (i) SUS has quasi-monopsonic power over general ambulatory services and hospitals; (ii) mental health specialists belong mostly to SUS; (iii) most of mental health services are outpatient ones; (iv) few CAPS have daily-bed available; and (v) there is very low federal financing to these innovations.
