Abstract:

Brazil has the second highest cesarean section rate in the world. There is a difference between these rates in the public and private sectors. Cesareans have higher health care costs. There are few studies focused on the private sector. Thus, we used hospitalization databeneficiaries living in the state of São Paulo, admitted between 2015 and 2021 with ages between 10 and 49 years to verify the rates and costs of cesarean sections in the private sector. A health economic evaluation was conductedthe perspective of supplementary health considering direct medical costs. We analyzed 757,307 hospitalizations with spending of R$7.701 billion. The cesarean rates were 80% in the period. They are lower in younger women (69%) and higher in older women (86%), always higher than 67%. This population has rates 71% higher than those of the SUS. There is a higher proportion of use of intensive care unit use in cesarean sections. The median cost of cesarean sections is 15% higher than normal deliveries (R$7,805 and R$6,807, respectively) and they are 2 times higher in insurance companies than in medical cooperatives. There is an opportunity to apply public health policies widely used in the Unified Health System to reduce fees, direct costs of hospitalization and health insurance.
