The aim of this article is the analysis of the implementation of mental health community services in the regional context in Brazil The article describes the accessibility to the Psychosocial Community Centers (CAPS) in the State of Rio de Janeiro in the current decade. Different categorical users of community services were identified through a cross-sectional survey. The theoretical models of organizational isomorphism and innovation and diffusion of policies were applied to discuss adoption and development of mental health community services in public sector.Despite the local context, the article demonstrates that the scale of commitment with patients is quite discrepant among community mental health services, especially those coming out of long-stay hospitalizations or hospitalized. It concludes that the analysis of different categories of users allows problematize the barriers of access to community services. The article also remarks that the identification of barriers to accessing CAPS can contribute to overcome weaknesses of current Brazilian mental health policy.
