Abstract:

Distinct models of health management reflect their founding conceptions and their institutional arrangement can lead to the health policy improvement. The present paper aims to make a reflection on the potentiality and limitations of the organizational structure and the social capital to produce changes on the performance of public health organizations searching for a better quality of care. The description and analysis of two experiences of universal public health systems, in Catalonia and in Brazil, show similarities of the legal basis in the health system, despite the diversity on the chosen paths, which originated distinct management experiences, one prioritizing the managerial organization and the other, the protagonism of social actors promoting the institutionalization of social capital. It was suggested that models of management with dialogue between an efficient organizational design and citizen participation capable of constructing social capital may promote changes on organizational culture to favor quality of care.
