Abstract:

The objective of this study is to evaluate the spatial distribution of live births (LB) of São Paulo Municipality (MSP), spatial dependence, identify possible differences in births profile and evaluate the distances between homes and delivery hospitals. We studied LB occurring in hospitals of high complexity, four Public Healthcare System (SUS) and four from the private network (non-SUS) in MSP in 2008. It was geocoded 46,190 LB: 48.8% from SUS hospitals and 51.2% from private hospitals, these accounted for 27.9% of total live births of MSP. The LB - SUS hospitals formed two clusters with a high proportion of households with incomes of ½ to 2MW, high concentration of slums and birth rates. The LB of non-SUS hospitals formed a cluster in the central region of MSP, where there is a high proportion of households with >10 MW income and low birth rate. The median distance (7.9 Km) between mother’s households and SUS hospitals is lower than that of private LB (9.4Km). Differences on maternal, pregnancy and childbirth characteristics were statistically significant, among SUS and non-SUS, as well as the frequency of multiple births. There was no difference in the prevalence of low birth weight and pre-term births, possibly due to live births occurred in hospitals high complexity.
