Abstract:

Decentralization and regionalization are strategic issues for health system reforms. The article analyzes the complex process of health regionalization production in Brazil, built in act by actors with different views on inter-federative relationship and health regions. It Identifies that the recent Unified Health System´s (UHS) normative framework advances towards its institutionalization and overcoming of the initial centrality in municipalization, strengthening the regionalization and the intergovernmental pact of health, but also highlights the need to promote its review. Starting with document analysis, literature review and the reflections of the authors, involved with UHS management and militant knowledge production, the challenges for the construction of a living regionalization are discussed: its relation to planning and dimensioning of network services, the production of live networks of care and shared management spaces, the inter-federative pacts and regional regulation, the coordination capacity of the regional system and funding, and the impact of the political dimension and electoral cycles. By understanding that regionalization (and UHS itself) is an open book, ways and possibilities on how to maintain a living regionalization are indicated.
