O objetivo deste estudo foi investigar a associação entre os determinantes sociais da saúde e os desfechos de acúmulo de biofilme e condição de mucosa bucal em idosos. Este estudo transversal, conduzido entre 2004 e 2005, incluiu 785 idosos de Carlos Barbosa, RS, Brasil. Questionários foram utilizados para coletar as variáveis referentes aos determinantes sociais da saúde. O exame físico das estruturas bucais e o Índice de Mucosa e de Biofilme de Henriksen (MPS) foram utilizados para avaliar os três desfechos: desfecho MS (mucosa bucal), PS (acúmulo de biofilme) e MPS (mucosa bucal + acúmulo de biofilme). Observou-se maior prevalência de acúmulo de biofilme (PS) moderado/grave entre os indivíduos mais velhos, do sexo masculino e com menor escolaridade. O uso de prótese dental total superior e renda de até 1 salário mínimo foram fatores de proteção para acúmulo de biofilme. Apenas as variáveis do primeiro bloco do modelo teórico, sexo masculino (OR = 2,13; IC95% = 1,26-3,61) e menor escolaridade (OR = 1,37; IC95% = 1,06-1,78), permaneceram associados como risco à presença de escore inaceitável do desfecho MPS, após análise multivariada. Os achados sugerem que idosos do sexo masculino e com menor escolaridade têm maior chance de apresentar saúde bucal precária.

The physical examination of the oral structures comprised the Mucosa-Plaque Index proposed by Henriksen (MPS) were used to evaluate the three outcomes: MS (buccal mucosa), PS (biofilm accumulation) and MPS (buccal mucosa + biofilm accumulation). A higher prevalence of moderate/severe plaque accumulation was observed in the older age group, in males and in less educated elderly. The use of full upper dentures and lower income were protective factors for moderate/severe plaque accumulation. Only male gender (OR = 2.13, 95% CI 1.26 to 3.61) and lower education (OR = 1.37, 95% CI 1.06 to 1.78) remained associated with an unacceptable MPS score. The findings suggest that older male and less educated are more likely to have poor oral health.
