Abstract:

Introduction − The implementation of electronic health record in healthcare services has reflected isolated solutions, which contributes to the fragmentation of Brazilian Health System. This research uses a strategic planning tool in order to investigate the path of implementation of the EHR in the National Health Institute a medium and high complexity hospital, reference in women and the children´s health. The objective was to outline possible contributions to the computerizing of health facilities in general. Methods − The methods are based on the ethnography of hospitals. The study design was qualitative, descriptive with documentary analysis. The sources consulted were the registry of the meetings held between 2012 and 2015. Results − A synthesis narrative points out the lack of infrastructure, resources and the conflict of interests involving several actors in the process. Discussion − The electronic record legitimizes the institutional design and organizes the work, besides mapping the hospital geography. Conclusion − The association of physical and digital records requires a change of institutional culture. Registration is an act of citizenship and becomes a key document in quality control and hospital accreditation.
