Abstract:

The objective was to verify the evolution of the Expanded Family Health and Basic Healthcare Centers (NASF-AB) implementation in the municipalities of the Southern region of Brazil,2008 to 2019, in light of the inverse equity hypothesis. Ecological study considering 1,188 municipalities in Southern Brazil. The analyzes were separated by state, with municipalities divided into Municipal Human Development Index - Income quartiles. The cumulative percentage of NASF-AB implementation was calculated and the inequality between Q1 (richest) and Q4 (poorest) verified by absolute and relative inequality measures. In Paraná, Q1 presented higher NASF-AB coverage than Q4, although inequality had reduced at the end of the period, it was still well demarcated, following a “top inequality” pattern. In Santa Catarina, what was predicted by the hypothesis occurred, with increase in inequalities at the beginning and subsequent reduction when NASF-AB already existed in about 90% of the municipalities in Q1, characterizing “botton inequality”. In Rio Grande do Sul, the hypothesis was refuted by observing,2014, greater implementation in Q4 compared to Q1. Public efforts should contribute to the inverse equity hypothesis is no longer a reality in the health services implementation.
